Please take about 20-30 mins to fill the form as they are a great guide for us to connect to what direction will help you best. Eastern Medicine is a whole body approach, hence the importance of the in-depth questionnaire and history.​​ If you do not have a printer, please arrive 20-30 mins early to your appt to fill out in person.
